Registered Nurse, Home Health

Ensures that all activities performed align with the vision of Elara Caring Maintains patient and staff privacy and confidentiality Assesses assigned case load of home health care patients and families Assumes primary responsibility for caseload Makes the initial evaluation and re-evaluates the patient’s nursing needs during each visit Communicates significant findings, problems, and changes in health condition Revises the plan of care in response to identified patient care issues Requirements Current, unrestricted state RN license 1+ year experience in a clinical care setting as a nurse Able and willing to travel within branch/office coverage area Ability to frequently lift, push, pull, and support up to 50 pounds Must have a dependable vehicle, valid driver’s license, and current auto insurance Associates Degree or Bachelor’s Degree in Nursing is preferred Previous Home Health or Hospice Experience is preferred Previous experience with HomeCare HomeBase is preferred Core Competencies Demonstrates expertise in patient assessment, care planning, and communication within home health care settings. Holds a current RN license and possesses experience in clinical care, particularly in home health or hospice environments.
