Responsibilities Deliver compassionate, skilled nursing care to patients wherever they call home. Collaborate with interdisciplinary teams to coordinate patient care and facilitate effective communication among providers and team members. Provide clinical, emotional, and educational support to families and caregivers. Advocate for patients by educating them about their conditions, treatment options, and available resources. Work closely with a team of professionals committed to dignified care. Advocate for comfort, respect, and quality of life. Qualifications RN license and clinical experience (hospice, palliative, geriatric, or home health preferred, but not required). A calm presence under pressure, strong listening skills, and a heart for hospice. Ability to work independently, maintain integrity, and collaborate with an interdisciplinary team.
