Job Summary

We are seeking an experienced Registered Nurse Case Manager to coordinate patient care, facilitate discharge planning, and ensure seamless transitions across the continuum of care. This role is ideal for a clinically strong RN with acute care experience who is passionate about patient advocacy, care coordination, and improving outcomes. Working closely with physicians, patients, families, and interdisciplinary teams, you will play a key role in developing individualized care plans and ensuring safe, effective discharge processes.

Qualifications

• Active Registered Nurse (RN) license required
• Associate Degree in Nursing or Nursing Certificate required
• Minimum 2 years of recent acute care experience within the last 3 years required
• Background in ICU, CCU, Medical Surgical, LTAC, or Case Management strongly preferred
• Strong critical thinking, communication, organizational, and patient advocacy skills

Compensation & Benefits

• Pay Range: $34.25 – $47.10 per hour (based on experience)
• Sign-On Bonus: Available on a case-by-case basis

Schedule: Full-Time - Day Shift
Hours: 8:00 AM – 4:30 PM
