Job Description Job Description PRIMARY JOB RESPONSIBILITIES Evaluates, interprets, plans, implements, and assesses patient care based on their age and medical condition. Efficiently oversees the care of a designated group of patients while collaborating with a multidisciplinary team. Communicates relevant information in line with organizational policies and discipline-specific guidelines. Contributes to the overall effectiveness of programs. JOB REQUIREMENTS Education: Must be a graduate from an accredited nursing program, holding either an Associate Degree, Diploma, or Bachelor of Science in Nursing. Licensure/Certifications: Must possess a current nursing license in the state of practice (NC/SC) and hold a valid CPR certification. Experience: A minimum of one year as a registered nurse and at least two years of clinical experience is required. Additional experience may include roles such as LPN, CNA, military medic, EMT, or similar. Home health experience is preferred. For those with less than one year of RN experience, one year of clinical experience as an LPN is necessary (internal use only). Therapy Assistants (PTA, OTA) with one year of home health experience and at least six months of RN experience may also be considered (internal use only). Technical/Motor Skills: Requires hand-eye coordination for administering injections, operating a computer, and handling patient care equipment. Must be proficient in English communication and literacy. Interpersonal Skills: Ability to establish positive interactions with patients, their families, physicians, and staff to ensure effective patient care. Physical Requirements: Must be capable of lifting or maneuvering objects weighing at least 50 pounds as part of patient care. Frequent physical activities include pushing, moving, and lifting patients, as well as assisting with bathing and ambulation, which requires significant physical effort. Occasional overhead reaching, stair climbing, and fine motor skills are also necessary. Mental Abilities: Must be able to assess patient conditions, develop care plans, choose suitable interventions, evaluate patient responses, and educate patients about their health and recovery. Requires strong cognitive skills, including both short- and long-term memory, the ability to prioritize tasks, and a willingness to learn new patient care techniques. Sensory Requirements: Ability to visually assess patients and utilize sight for implementing and evaluating care plans (e.g., changing dressings, starting IVs). Must also use hearing to auscultate lung and bowel sounds, respond to alarms, and communicate effectively with patients, families, and staff. Hazard Exposure: Potential exposure to noise, bloodborne pathogens, body fluids, infectious diseases, and needle injuries. May encounter traffic hazards and dangerous animals during home visits, as well as situations that could pose a threat to personal safety. Must be prepared for varying temperatures and weather conditions. Work Hours: Variable schedule from Monday to Friday, with availability for weekends and holidays as required. Flexibility is essential to meet staffing needs. Population Served: Primarily adults and geriatric patients. Additional Requirement: A valid driver's license from North Carolina and access to a reliable vehicle are necessary.
