PT - Physical Therapist

Outpatient Physical Therapist

This is a 13-week contract position.

5 days a week, 8 hours per day.

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patient's needs, goals, and physician direction. This role is responsible for the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that facilitate an effective return to community activities. The therapist serves as an educator for patients, families, students, coworkers, and themselves while upholding the mission and core values of the organization.

Responsibilities:

• Completes comprehensive evaluations and develops treatment plans focused on patient discharge within established length of stay.
• Document medical records clearly, accurately, and promptly, ensuring compliance with state, federal, and insurance requirements.
• Participate in student programs and provide a positive learning environment for students.
• Create individualized professional development plans and pursue continuing education actively.
• Adhere to state practice acts and maintain professional licensure in good standing.
• Demonstrate awareness of organizational performance improvement initiatives and follow through with action plans.

Required Experience / Certifications / Licensure:

• Degree in Physical Therapy from an accredited school.
• 6 months to 1 year of outpatient clinical experience.
• Current CPR certification.
