Registered Nurse - Hospice

$51 - $53 per hour

Position Overview Seeking an experienced and compassionate Hospice Registered Nurse to provide comprehensive case management for patients receiving end-of-life care. This role is responsible for managing a full caseload, coordinating all aspects of care, and ensuring patients and their families receive high-quality, supportive services throughout the hospice journey. Schedule & Work Details Location: Oklahoma service area Schedule: Monday through Friday, 8:00 AM to 5:00 PM Weekly Hours: Full-time schedule Productivity Expectation: 6 visits per day On-Call/Weekend Requirement: Back-up on-call rotation every 5th week Mileage Reimbursement: Yes, reimbursed between patient visits (first patient to last patient) Pay Rate: $51 to $53 per hour Key Responsibilities Manage a caseload of hospice patients as the primary RN case manager Conduct comprehensive patient assessments and ongoing evaluations Develop, implement, and update individualized Plans of Care (POC) Coordinate all patient care services with the interdisciplinary team Order medications, medical supplies, and equipment as needed Provide symptom management, pain control, and end-of-life care Educate patients and families on disease progression, care needs, and available resources Maintain timely and accurate clinical documentation Communicate effectively with physicians and care team members to ensure continuity of care Participate in on-call rotation to support patient needs after hours Required Qualifications Active Registered Nurse (RN) license in the state of Oklahoma Prior hospice nursing experience required Strong clinical assessment and case management skills Ability to manage multiple patients and prioritize effectively Valid driver's license and reliable transportation Ability to travel within assigned service area Preferred Qualifications Previous experience in hospice case management Strong knowledge of hospice regulations and best practices Excellent communication and interpersonal skills Compassionate and patient-focused mindset Strong organizational and time management skills Experience with electronic medical records (EMR) systems Benefits Pride-Health offers eligible employee's comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance and hospital indemnity), 401(k)-retirement savings, life & disability insurance, an employee assistance program, legal support, auto, home insurance, pet insurance and employee discounts with preferred vendors.
