Oasis Therapy Services is seeking a Speech-Language Pathologist (SLP) to provide one-on-one rehabilitation services at Willow Falls in Crest Hill and Eden Vista Rock Run in Joliet, Illinois.

EMR: Net Health
Responsibilities
- Evaluate and treat communication, cognitive, voice, and swallowing disorders.
- Develop individualized treatment plans with measurable goals.
- Monitor progress and adjust treatment plans as needed.
- Educate residents and caregivers on communication strategies and swallowing precautions.
- Complete accurate Net Health documentation and collaborate with the care team.
Qualifications
- Active and current Illinois Speech-Language Pathologist license.
- Master's or doctoral degree from an accredited Speech-Language Pathology program.
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred.
- Current CPR certification.
- Strong communication, organization, and documentation skills.
- Geriatric, dysphagia, skilled nursing, or rehabilitation experience is preferred.
- Reliable transportation and ability to travel between Crest Hill and Joliet.
