Physical Therapist (PT) - Outpatient

Full-time Physical Therapist specializing in outpatient rehabilitation for community and facility-based patients, primarily older adults, in Northampton, MA. Responsibilities Provide patient-centered in-home outpatient therapy, assessing needs and developing individualized treatment plans. Perform evaluations, instruct patients and caregivers, and make referrals to other disciplines as needed. Implement therapeutic techniques including therapeutic exercises, manual therapy, modalities, and adaptive device training. Supervise support personnel, ensure documentation accuracy, and adhere to treatment schedules. Maintain a safe environment, order supplies, and perform routine equipment maintenance. Participate in team meetings, case management, and community mobility assessments. Qualifications & Skills Valid Physical Therapy license in applicable state(s) or proof of eligibility. Master’s degree in Physical Therapy or equivalent; or Bachelor’s plus 5 years of experience. Reliable transportation and valid driver’s license.
