Licensed Practical Nurse / LPN - Hospice-The Licensed Practical Nurse assists in planning, coordination and care of hospice patients in accordance with the patient’s plan of care. The position provides direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the interdisciplinary team to help achieve positive care outcomes.NOTE: Schedule is M-F full-time 40 hours with some additional after hours/weekend back up shifts.Coverage area is Tigard, Tualatin, King City, and Wilsonville areas.Estimated Hiring Range:$33.98 - $41.53Bonus Target:Current CareOregon Employees: Please use the internal Workday site to submit an application for this job.-Essential ResponsibilitiesProvide direct patient care as defined in State Nurse Practice Act following a patient assessment and an approved plan of care initiated by the registered nurse. This position may be responsible for one or more of the following areas:Focused/limited nursing visitsTelephonic triageNight/weekend coverage back up rotation as shared among the nursing team in case the primary nurse is offOther duties assigned by the Hospice LeadershipCollaborate with hospice team members regarding patient care.Perform focused/limited nursing assessment, recognize signs and symptoms of end-stage medical conditions/illnesses and implements planned nursing and medical interventions as delegated by registered nurse/clinician.Perform duties in compliance with Medicare Condition of Participation.Responsible for gathering and reviewing documentation in the electronic medical records, assuring accuracy, completeness and compliance with regulations, and certification standards.Recognize signs and symptoms of common and routine medical conditions/illnesses, abnormal versus new/existing and implements planned nursing and medical interventions as delegated by registered nurse/clinician.Administer medications and treatments as prescribed by patient’s prescribing clinician. Evaluate patient’s response to treatments/medications and notify hospice providers and/or registered nurses of concerns.Contribute to the development, implementation, or modification of the individualized patient's plan of care under the direction of the registered nurse/clinician.Provide health teaching/education and support to patients and caregivers related to end-of-life care needs.Assist with hospice chart auditing, as requested.Experience and/or EducationRequiredGraduate of an accredited LPN training programMinimum one (1) year of experience as a licensed practical nurse in home health, hospice, skilled care facility, memory care unit, or acute care setting (preferably medical/surgical)Current unencumbered LPN license in the State of OregonCPR certification at hire or within 6 months in positionValid driver’s license, acceptable driving record, and automobile liability coverage or access to an insured vehicleKnowledge, Skills and Abilities RequiredKnowledgeKnowledge of Federal, State and Local regulations, Hospice Medicare Conditions of Participation, and HIPAA privacy rules and regulationsSkills and AbilitiesDemonstrate nursing abilities and counseling in a hospice settingPractice within the scope of license, certification or registration, where applicable, and as required by law and regulationMust have general computer skills and a working knowledge of Electronic Medical Records systems, MS office, Outlook, and the internetExcellent interpersonal skillsAbility to tolerate and manage stress and griefAbility to work both independently and collaborativelyAbility to work under pressure to meet deadlinesAbility to take initiative and utilize innovative techniques and ingenuityAbility to interact with various departments throughout the organizationAbility to work flexible hours, if neededAbility to plan, prioritize and coordinate multiple projects or tasksAbility to adhere to organizational standards, policies and proceduresAbility to work effectively with diverse individuals and groupsAbility to learn, focus, understand, and evaluate information and determine appropriate actionsAbility to accept direction and feedback, as well as tolerate and manage stressAbility to see, read, hear, and speak clearly for at least 6 hours/dayAbility to stand, walk, sit, and perform repetitive finger and wrist movement for at least 3-6 hours/dayAbility to lift, carry, push, pull, climb stairs, bend, and/or pinch small objects for at least 3 hours/dayAbility to operate a motorized vehicle in all kinds of weather conditionsWorking ConditionsWork Environment(s): Indoor/Office Community Facilities/Security Outdoor ExposureMember/Patient Facing: No Telephonic In PersonHazards: May include, but not limited to, physical, ergonomic, and biological hazards.Equipment: General office equipment and/or mobile technologyTravel: Requires travel outside of the workplace at least weekly; the employee’s personal vehicle may be used.
