Job Title: Registered Nurse Profession: Nursing Specialty: Case Manager Duration: 12 Weeks Shift: Day (Monday - Friday) Hours per Shift: 8 Hours (08:00 - 16:30) Experience: Minimum of 3 Years License: GA or Compact License Required Certifications: BLS Must-Have: - Ability to assess, evaluate, and drive discharge plans. - Experience in care coordination and discharge planning. - Knowledge of Medicare regulatory guidelines regarding patient choice. - Familiarity with concurrent reviews and medical necessity criteria. Description: The role involves assessing and evaluating patients to facilitate effective discharge planning. You will coordinate services to ensure patients receive the appropriate level of care. It is essential to initiate, evaluate, and re-evaluate treatment plans daily. You will monitor patient progress and identify barriers to discharge. Collaboration with interdisciplinary teams is critical for successful outcomes. Compliance with CMS, HIPAA guidelines, and other regulatory standards is mandatory. Attention to detail in documentation and adherence to care protocols are required. This position involves managing patient caseloads effectively within established benchmarks. The candidate should have experience in acute hospital settings. Interest in optimizing length of stay and patient satisfaction is a plus. You are expected to work during major holidays if they occur within your assignment period.
