Social Worker - Case Management

Schedule: - Shift: 8-hour days - Weekend requirement: No
Assignment Details: - Visit expectations: May see up to 6 patients/day - Guaranteed hours: Facility may call off up to one shift per pay period
Requirements: - License: LMSW (pending accepted) - Certifications: BLS (BCLS) - Years of experience: 1 year skilled home health experience - Required clinical experience: Oasis experience required - Charting systems: Epic (EPIC Remote Client for Home Health), Rover app
Roles & Responsibilities: - Conduct psychosocial assessments for patients. - Coordinate care and manage cases effectively. - Develop and document plans of care according to patient needs. - Assist with discharge planning for patients transitioning from care. - Ensure compliance with Medicare/Medicaid regulations.
Compliance / Notes: - Attire: Business casual (scrubs allowed; no jeans) - Traveler must provide own transportation; daily drive radius may be up to ~60 miles. - COVID vaccination requirement: Confirm with hiring manager. - References: 2 references required covering 1 year (within last 3 years).
