RN Case Manager Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans
• Provide skilled nursing visits in accordance with physician orders
• Monitor patient progress and evaluate physical, psychosocial, environmental, and caregiver needs
• Coordinate care with physicians, therapists, social workers, and other healthcare professionals
• Communicate changes in patient condition and recommend appropriate interventions
• Educate patients and caregivers regarding treatment plans and disease management
• Maintain accurate and timely clinical documentation
• Ensure compliance with regulatory standards and patient confidentiality requirements

Qualifications

• Current, active, unencumbered Registered Nurse (RN) license
• Minimum of one year of nursing experience
• Valid driver's license, reliable transportation, and auto insurance
• Ability to travel throughout the assigned territory
• Home Health experience preferred
• Strong communication, organization, and case management skills

Compensation & Benefits

• Salary range of $85,000 - $98,000 based on experience
