Occupational Therapist (OT) - Consistent PRN Location: Elroy, WI Facility: Elroy Health Services Setting: Skilled Nursing Facility (SNF) Join the Continuum Therapy Partners Rehab Team at Elroy Health Services! Continuum Therapy Partners is seeking a compassionate and dedicated Occupational Therapist (OT) for a consistent PRN opportunity at Elroy Health Services in Elroy, Wisconsin. This PRN Occupational Therapist position offers flexibility while providing meaningful patient care in a supportive, collaborative skilled nursing facility where your clinical expertise makes a daily impact. As an Occupational Therapist (OT) , you will work with adult and geriatric patients to improve independence, safety, mobility, and quality of life through individualized occupational therapy interventions. Responsibilities Evaluate patients and develop individualized occupational therapy treatment plans. Provide skilled occupational therapy services in a skilled nursing facility setting. Address activities of daily living (ADLs), functional mobility, coordination, strength, cognition, and safety awareness. Help patients improve independence and participate in meaningful daily activities. Monitor patient progress and adjust treatment plans based on patient needs. Collaborate with physicians, nursing staff, caregivers, and interdisciplinary rehabilitation team members. Complete accurate and timely documentation according to company, state, and federal guidelines. Educate patients and caregivers on therapeutic techniques, adaptive equipment, and safety strategies. Provide compassionate, patient-centered occupational therapy services. Qualifications Graduate of an accredited Occupational Therapy program. Current Wisconsin Occupational Therapist (OT) license required. Skilled nursing or rehabilitation experience preferred but not required. New graduates are encouraged to apply. Strong communication, organization, and teamwork skills. Passion for providing high-quality, patient-centered care.
