Responsibilities

• Provide direct patient care and support under the supervision of registered nurses.

• Administer medications and monitor patient responses to treatments.

• Assist in developing and implementing patient care plans.

• Participate in ongoing professional development and training opportunities.

• Engage in performance measurement and strive to meet established targets.

• Document patient progress and maintain accurate medical records.

• Collaborate with healthcare team members to optimize patient outcomes.

• Ensure compliance with healthcare regulations and safety standards.

• Educate patients and families on health management and wellness.

• Monitor vital signs and report any significant changes to the nursing staff.

• Assist with daily living activities to enhance patient comfort.

• Maintain a clean and safe environment for patients and staff.

Experience/Qualifications

• Current LPN licensure in the state of employment.

• Commitment to ongoing professional development and training.

• Experience in providing direct patient care in a clinical setting.

• Ability to engage patients and families in health education.

• Strong understanding of medication administration and monitoring patient responses.

• Familiarity with healthcare regulations and safety standards.

• Willingness to monitor vital signs and report changes promptly.

• Ability to collaborate effectively with a multidisciplinary healthcare team.

• Strong documentation skills for maintaining accurate patient records.

• Experience in assisting with daily living activities for patient comfort.
