Case Manager RN

Job Summary As a Case Manager RN with TriStar Horizon Medical Center you will support patients and families through their care journey. You will coordinate services, connect resources, and develop individualized care plans. By partnering with physicians, nurses, and department leaders, you will help ensure safe transitions, clear communication, and consistent quality across the continuum of care.
