As a Social Worker, you will advocate for the well-being of our residents and ensure their holistic needs are met. Your work will play a pivotal role in enhancing the quality of life of our residents. You will also be responsible for discharge planning, helping to coordinate safe, smooth, and well-supported transitions of care. This includes collaborating closely with residents, families, and the interdisciplinary team to ensure continuity and appropriate post-discharge resources.

Key responsibilities of a Social Worker include:

• Conducting comprehensive psychosocial assessments to identify residents' social, emotional, and environmental needs, as well as strengths and preferences
• Collaborating with residents, their families, and the interdisciplinary team to develop individualized care plans that address social, emotional, and psychological concerns
• Providing counseling and emotional support to residents and their families, offering guidance on coping strategies, adjustment to care transitions, and decision-making processes
• Serving as a strong advocate for residents' rights and preferences, ensuring their voices are heard and respected in care planning and decision-making processes
• Connecting residents and their families with community resources, support services, and advocacy organizations to address social, financial, and legal needs
• Providing crisis intervention and support during challenging situations, such as grief and loss, family conflicts, or changes in health status

Qualifications of a Social Worker include:

• Master's degree in social work from an accredited program
• Current licensure or certification as a Social Worker in the state of employment
• Previous experience in Long-Term Care, geriatrics, or a related field
• Strong assessment, counseling, and crisis intervention skills
• Excellent communication and interpersonal abilities
• Ability to work collaboratively as part of a multidisciplinary team
• Empathy, compassion, and a commitment to person-centered care
