Social Worker MSW, Inpatient

Piedmont Healthcare is seeking an MSW Social Worker for our inpatient team. In this role, you will provide psychosocial assessment, counseling, and crisis intervention to hospitalized patients and families, coordinating care plans that support safe discharge and continuity of care. Partner with physicians, nurses, and community agencies to address social determinants of health, arrange post-acute services, and advocate for patient needs. You will maintain accurate documentation, participate in interdisciplinary rounds, and support quality, patient-centered care in a collaborative, inclusive environment with opportunities for professional growth.
