This role promotes Resident Centered Care by addressing psychosocial needs, supporting families and ensuring compliance with all federal, state and local regulations while upholding Sunrise's Mission and Principles of Service.What You'll Do:Assess residents' psychosocial needs and document care plans per regulatory requirementsLead and participate in family and care plan conferencesProvide counseling and one-on-one support to residents and familiesCoordinate admissions, discharges and room transfersComplete and support MDS and care planning processesAct as liaison with community agencies and external resourcesSupport Resident Council and coordinate transportation as neededEnsure compliance with social services regulations and Sunrise standardsManage departmental operations, staffing, scheduling and engagement initiativesPartner with leadership on budget management and financial oversightPay Range: 30.00 - 40.00Ready to take the next step and make a bigger impact than you ever imagined?
