Clinic LVN (Licensed Vocational Nurse)

Job Description Job Description Vivo HealthStaff is recruiting for Clinic LVNs in San Marcos, California for a large geriatric healthcare organization. The Clinic LVN is responsible for coordinating and providing direct and indirect nursing care to patients at the center as well as at the patient's home. They will work in close coordination with the Physician, Registered Nurse, clinic staff, and other interdisciplinary team (IDT) members to manage ongoing care, complex clinical situations, and support the operation of the primary care clinic. Essential Functions • Works under the supervision of the Clinic Director/Registered Nurse and maintains vocational nursing professional standards of care. • Performs home visits to administer prescribed medications and treatments in accordance with nursing standards to supplement clinic services. • Observes, records, and reports patients' condition and reaction to medication and treatments to physicians. • Documents all interactions related to clinic visits in the electronic medical record. • Instructs patients and family members regarding medications and treatment instructions in the home by CNAs, PCWs, and homemakers. • Keeps Home Care Supervisor and Interdisciplinary team informed on home care issues of patients. • Attends morning meetings at the center, family meetings, and any other meetings as assigned. • Completes all required documentation in a timely and accurate manner. • Protects privacy and maintains confidentiality of all company procedures and information about employees, patients, and families. • Follows policies and procedures and participates in any required Quality Improvement activities, staff training, and meetings. • Performs other duties as assigned. • Requires the use of a personal vehicle. Position Description Education and Certification • Graduate of an accredited school of nursing. • Unencumbered California Licensed Vocational Nurse (LVN) License. • Current Basic Life Support (BLS) / First Aid Certification required. Skills & Experience • Minimum of two years nursing experience in a clinical setting with a frail or elderly population. • Nursing knowledge and skills necessary to treat frail, elderly patients and manage complex clinical situations. • Highly motivated, self-directed, able to execute tasks in a quickly changing environment and make sound decisions in emergency situations. • Excellent clinical, organizational, and communication skills in settings with seniors, their families, and interdisciplinary team members. • Experience and competency working with people from diverse backgrounds. • Commitment to unlocking the full potential of our most vulnerable seniors. • Bilingual Spanish-English preferred.
