Our Care Coordination team provides our therapist with valuable resources such as scheduling and documentation support so that you are able to focus on what matters most, patient care. Job Responsibilities: Supervision to Asst. therapy staff Communicating daily with office staff to accept new patients, complete paperwork and turn in payroll Keeping open communication with patients and home health agencies to promote a positive and caring experience Facilitating communication with Physical Therapist Assistant to manage patient care Completion of Evaluation, Plan of Care, Reassessment, and Discharge visits and paperwork Attend meetings and case conferences Job Requirements: Current Physical Therapist Licensure Bachelor’s degree in Physical Therapy Ability to travel to patient’s home or facility Home healthcare experience preferred This position requires daily commute to patients' homes and facilities in a 40-mile radius of Lorain, OH #J-18808-Ljbffr ProHealth Partners Inc.
