Care Manager RN

Description Care Manager RN at Petaluma Valley Hospital in Petaluma, CA. This position is Per Diem and will work 8 hour, Day shifts. Utilizing a collaborative process, the Case Manager will establish a continuity of care plan for a caseload of patients. The Case Manager will establish quality monitoring indicators, collection of data, analysis, documentation and the reporting outcomes, and liaison with patients. Required Qualifications: Upon hire: California Registered Nurse License 2 years of Acute care clinical or comparative experience. Preferred Qualifications: Bachelor's degree in nursing.
