Licensed Practical Nurse

Position Summary The Licensed Practical Nurse provides quality nursing care to residents, coordinates total nursing care, collaborates with the Director of Nursing and Unit Managers, ensures compliance with policies, and may perform duties of a Certified Nurse Aide as required. Responsibilities Assess resident conditions, develop Care Plans, record observations, and implement interventions. Conduct regular rounds with physicians, observe symptoms, and adjust treatments. Provide preventative, supportive, maintenance, and rehabilitative care addressing physiologic and psychosocial needs. Perform treatments including dressing changes, wound care, Foley catheter irrigation, IV therapy, tube feedings, chest tubes, and peritoneal dialysis. Ensure supplies and equipment are available; schedule appointments, therapies, and consults. Document in the appointment book and maintain accurate records. Assist CNAs with resident care duties such as bathing, feeding, and ambulation. Administer medications per code and policies; report contraindications. Advise residents/external parties on medication changes and treatment plans. Manage and document emergency care, coordinating communications with physicians, DON, administrators, and emergency services. Conduct assessments during admission, transfer, and discharge; complete summaries. Perform quality improvement activities and monitor accident and infection rates; propose corrective actions. Weekly skin assessments and report skin condition data. Report complaints, unusual occurrences, and abuse allegations promptly. Participate in regulatory surveys and prepare Plan of Correction responses. Report resident injuries to Unit Manager. Understand and enforce residents’ rights, including restraints and abuse policies. Support facility philosophy, standards, and policies. Communicate with department heads to solve problems and plan effective care. Notify Director of Nursing of staffing shortages. Present a professional appearance as a unit role model. Maintain professional development through seminars and education. Promote safety protocols to prevent injury and reduce workers’ compensation costs. Ensure equipment is used safely and report maintenance issues. Attend staff meetings and in‑services as requested. Perform other tasks as required. Required/Desired Qualifications Licensed Practical Nurse with a current Michigan license and CPR certification. Two years of professional nursing experience in a long‑term care setting is preferred. Specific Skills, Knowledge, and Abilities Knowledge of federal and state nursing laws and regulations for long‑term care. Strong organizational, time‑management, and prioritization skills. Ability to apply math concepts such as fractions and ratios. Basic computer knowledge. Other Special Requirements Tolerate frequent exposure to blood, bodily fluids, hazardous materials, chemicals, latex, and plastics. Manual dexterity required for modern office equipment.
