Job Details Job Location: CHI - Modesto - Spyres - Modesto, CA 95356 Salary Range: $50.00 - $67.20 Hourly Job Shift: Day At Community Hospice & Health Services and its affiliates, Community Hospice & Health Services Foundation, and CHI Management, Inc. DEPARTMENT: Patient Care Team (various) REPORTS TO: Chief Clinical Officer (CCO) SUPERVISES: Patient Care Teams SUMMARY The PCM is a registered nurse, duly licensed in California, who is responsible for the ongoing management of the patient care team to ensure the delivery of quality patient care, as called for in the Plan of Care (POC) adopted by the Interdisciplinary Team. RESPONSIBILITIES Manages patient care team staffing on a 24-hour basis, including on-call and weekend coverage to meet changing organizational needs and assure quality patient/family care. Reviews and conducts performance appraisals of staff on a regular basis. Is responsible for ensuring that hospice services, documentation of those services, and clinical care is provided according to professional standards and in compliance with all current regulations and conditions of participation. Reviews and revises policies and procedures manuals, relevant to patient care issues, on an ongoing basis as required to meet changing needs and to assure compliance with current regulations. Participates in the recruitment, hiring, retention, orientation, and ongoing training of nursing personnel; this may include personnel who are orienting to other areas of Community Hospice, as well as those who will be assigned to case management. Facilitates communication between the Interdisciplinary Team and demonstrates good problem-solving skills while utilizing a team-based approach. Coordinates communication and services with contracted companies, (e.g., Durable Medical Equipment [DME], infusion services, pharmacy, etc.) to ensure that patient care and patient/family teaching standards are met. Provides administrative on-call function on a rotational basis. Participates in Performance Improvement Strategies Committee and other committees as assigned. Responsible for maintaining a safe working environment for all hospice staff. Performs job duties in a professional manner and maintains confidentiality of patient information at all times. Must maintain RN license in accordance with federal, state and local laws, and must act only within the scope of his or her state license. All personnel qualifications must be kept current at all times. Participates and provides in-services, workshops, and clinical meetings to promote professional growth and understanding of hospice care for both CHI staff as well as community partners. Follows, enforces and monitors staff compliance with all Community Hospice policies and procedures including, but not limited to, safety, infection control, privacy and confidentiality, and HIPAA. Fosters relationships and communication with facility management to promote coordinated, comprehensive continuum of quality of care to hospice patients. Other Functions Participates in committees as requested by the Board of Directors, President/Chief Executive Officer/Administrator and/or the CCO. Administrative duties as requested by the Board of Directors, President/Chief Executive Officer/Administrator and/or the CCO. Provides education, utilizing a number of different media, regarding hospice services to patients, families, staff, and the community as needed. Responsible for establishing and maintaining a safe environment for patients/families, and a safe working environment for Community Hospice staff. Performs job duties and interacts with patients/families, Community Hospice staff, and community members in a courteous and professional manner. Maintains all required confidentiality and privacy requirements of both the organization and the regulatory agencies that have oversight of and for Community Hospice. The job demands that the PCM also be able to perform all of the routine duties of a Registered Nurse in a home-like environment (including those living in facilities) when needed. Performs all duties as assigned by the CCO, including work in other areas, to cover absences or relief to equalize peak work periods or otherwise meet the needs of the organization. Nursing Skills Commitment to principles, practices and procedures of nursing in a hospice environment. Demonstrated competence in hospice care delivery techniques, with willingness to learn ‘Most Current hospice nursing techniques and procedures. Dedication to ’Nursing Practices Act’ record keeping requirements. Routine nursing skills practiced in a clinical environment. Intolerant of conditions that are symptomatic of neglect or abuse. Patient Interpersonal Skills Sensitivity to and ability to interact with various racial, religious, and ethnic cultures in the organization service area. Ability to solicit sensitive information from patients and their families/caregivers. Ability to remain calm with patients and their families/caregivers under fast paced, emotional conditions. Explain complex ideas and procedures so patients and their families/caregivers can understand the issues related to hospice care. Management Skills Use of modern personnel management techniques. Business/clinical information/assessment gathering and recording techniques. Providing accurate and timely records of critical events. Cooperation with the Board of Directors, President/Chief Executive Officer/Administrator, CCO, physicians, IDT members, and other Community Hospice coworkers. Read, write, and communicate on a professional level. Perform multiple tasks in a timely manner. Perform efficiently in a high pressure setting. Deal effectively with various personalities on a routine basis and in a professional manner. Develop and maintain a professional relationships with coworkers. Follow written and verbal directions while retaining the ability to tactfully question directions that seem unclear, unsafe, or illegal. Identify and solve problems encountered in end-of-life care. Collect data at a level sufficient to meet Medicare Conditions of Participation mandates and California Hospice license standard and Interpretive Guidelines requirements. Meet deadlines and remain on timelines. Routinely work without supervision from the CCO. Qualifications Current California license to practice as a registered nurse. BSN status is preferred. One year of acute medical/surgical experience, skilled facility experience or equivalent preferred. Current CPR at the Basic Cardiac Life Support (BCLS) level. Current home health, hospice, or community health experience of two years is preferred. Certification as a Hospice and Palliative Nurse is preferred. One year of home health, hospice, or SNF management experience is preferred. Computer knowledge and experience working with electronic patient records, as well as various other applications. Recommended minimum WPM of 23. Must be able to maintain professional attitude, while managing multiple tasks, in a faced paced, emotional environment. WORK ENVIRONMENT This position works in an office environment using a computer and general office equipment. WORK SCHEDULE Position is regular full-time required to work a regularly scheduled workweek of at least 40 hours during regular business hours of the department managing, with overtime as needed to meet the needs of the organization. #J-18808-Ljbffr Community Hospice & Health Services
