Home Care experience is desired.Essential FunctionsPerforms assessments and develops individualized patient care plans and manages multi­ disciplinary care as applicable.Provides skilled nursing care in a patient's home as ordered by the attending physician in conjunction with the plan of care.Supervises LPN's and Home Health Aide's in a patient's home.Complies with agency policies and procedures. Promotes and maintains an agency environment that is in compliance with federal, state, and local regulations.Submits clear and appropriate documentation in a timely manner and documents patient education.Communicates effectively to physicians, supervisors, and coworkers in a timely manner.Collaborating with doctors and other healthcare professionals to develop improved diets and healthcare plans for patients.
