Collect data on residents’ physical and psychosocial status, document and communicate findings to the RN and other members of the Interdisciplinary Team. Contribute to the development of an evidence‑based, age‑appropriate care plan reflecting nursing diagnoses and interventions; involve residents, families, and the healthcare team to achieve desired outcomes. Recommend revisions to the plan of care after evaluating resident outcomes; initiate referrals to address all aspects of resident care as needed. Collaborate with residents/families and interdisciplinary team to ensure care is safe, effective, and culturally appropriate; promote a healthy and secure care environment. Maintain current knowledge and clinical competency; participate in mentoring or orientation of other staff; provide leadership under the direction of the RN. Adhere to established policies and procedures; protect staff and resident safety; comply with safety standards for potential exposure to bloodborne pathogens.
