LTSS Care Manager Compensation Competitive hourly pay Benefits available to full-time employees after 90 days 401(k) with company match available for eligible employees LTSS Care Manager Highlights Remote position supporting Region 2 counties in North Carolina Standard schedule: Monday – Friday, 8:00 AM – 5:00 PM EST 40 hours per week No overtime required Possibility for contract extension Potential for conversion, though not guaranteed Primarily remote role with face-to-face member visits Travel may exceed 50% depending on member needs Occasional in-person team meetings LTSS Care Manager Responsibilities Develop, assess, and coordinate holistic care management activities for members with complex medical needs Create and support individualized service plans for long-term care members Monitor care delivery and coordinate services across the healthcare continuum Educate members, families, and caregivers regarding available benefits, services, and community resources Collaborate with care managers, coordinators, navigators, utilization management, quality teams, and transition of care teams Connect members with providers and community-based resources Support quality, cost-effective healthcare outcomes through proactive care coordination Conduct member assessments and ongoing care plan evaluations Maintain accurate documentation and compliance with program requirements LTSS Care Manager Requirements Bachelor's degree required Active RN or LCSW license required Minimum of two (2) years of LTSS and/or HCBS coordination experience Experience with care delivery monitoring and care management Prior experience in social work, geriatrics, gerontology, pediatrics, or human services Strong communication and relationship-building skills Strong computer and documentation skills Ability to travel throughout assigned North Carolina Region 2 territory Preferred Qualifications Prior managed care experience Experience working with Medicaid populations Knowledge of community-based resources and long-term services and supports Top Skills Needed Care Management Experience Effective Communication Skills Computer and Documentation Skills Why You'll Love This Opportunity Make a direct impact on members with complex healthcare needs Work within a collaborative and supportive care management team Gain valuable experience in long-term care coordination and service planning Help improve healthcare outcomes and quality of life for vulnerable populations Build expertise in managed care, LTSS, and community resource coordination Apply today to be considered for this rewarding LTSS Care Manager opportunity serving members throughout North Carolina.
