Registered Nurse (RN) - Home Health - $72K-117K per year

Position: Registered Nurse (RN) – Home Health – Saint Louis, Missouri Schedule: Flexible schedule with full‑time, part‑time and PRN shifts available. Location: St. Louis Metro and North County regions. Responsibilities Assess patient preferences and barriers to involvement in care, including values, emotional, spiritual, cultural and population‑specific needs. Develop, implement, and document individual plans of care with defined goals in collaboration with the interprofessional team and the patient, family or caregiver, and proactively plan and ensure communication of the plan of care across the continuum of care. Promote respect, equity and empathy in interactions with diverse and vulnerable populations through care delivery (e.g., support for emotional, spiritual, and cultural preferences of patient, family and/or caregivers) and practice collaborative problem solving, service recovery and advocacy for patient and family‑centered continuity of care. Implement care by integrating data from the interprofessional team and applying critical thinking in a safe and timely manner; evaluate changes in patient condition, inform and collaborate with family or caregivers, and communicate changes with the interprofessional team while updating the plan of care in the EMR. Evaluate current nursing care to ensure evidence‑based practice and quality patient outcomes. Minimum Requirements Education: Nursing Diploma/Associate’s – Nursing (Experience Supervisor Experience: None. Licenses & Certifications: Valid driver’s license; Registered Nurse (RN).
