NOW HIRING a Physical Therapy Assistant for per diem OR part-time opportunities in the home care setting. You will be paid for each completed patient visit (with documentation). WORK INDEPENDENTLY * 1-to-1 IN-HOME PATIENT CARE Are you a dedicated individual with a heart for compassion and kindness? If you are, we want you to join our team! RESPONSIBILITIES: Perform home visits for patients in a home care setting Under the supervision of the Physical Therapist, performs treatment modalities as indicated according to the plan of care Implements plan of care to provide safe, efficient quality care to patients on a daily basis, reporting any changes to the Physical Therapist or Clinical Manager. QUALIFICATIONS: Current license as a Physical Therapy Assistant in Connecticut Valid driver’s license Reliable transportation and willing to travel in New London and/or Windham and/or Middlesex Counties Strong communication and interpersonal skills Home health experience including Medicare preferred BENEFITS: Flexible schedule Mileage paid between patient visits Training, professional development, and technology WHO WE ARE: At CareCo Medical Home Health, we've provided quality in-home support services since 2004.
