Schedule:

• Flexible shifts available Monday through Sunday
• Enjoy the freedom to create your own work schedule
• Commit to a minimum weekly or bi-weekly hourly requirement

Responsibilities:

• Conduct in-home risk assessments for Medicare Advantage members, including annual health evaluations and post-discharge visits
• Review and evaluate patients current and previous medical histories, ongoing conditions, and medication regimens
• Educate patients on their medical conditions and prescribed medications to promote understanding and compliance
• Identify gaps in care and recommend actionable steps for resolution
• Focus is strictly on assessmentno prescribing or development of treatment plans necessary

Qualifications:

• Active Nurse Practitioner license in New Mexico
• Board Certification through AANP or ANCC
• Current CPR certification (BLS)
• MSN from an accredited school of nursing
• At least six months recent experience as an NP (preferred)
