The LMSW supports patients and families by addressing the emotional, psychosocial, financial, cultural, spiritual, and environmental needs associated with end-of-life care while promoting comfort, dignity, and quality of life. This PRN position provides coverage as needed, including vacations, increased patient census, and staff absences. Services may be provided in: Private residences Assisted Living communities Independent Living communities Skilled Nursing Facilities Residential Care Facilities Other healthcare settings as assigned Essential Responsibilities Complete comprehensive psychosocial assessments for hospice patients and families. Evaluate emotional, social, financial, cultural, and environmental factors impacting care. Develop individualized care plans in collaboration with the interdisciplinary team. Provide counseling, emotional support, grief counseling, and crisis intervention. Assist patients and families with advance care planning and advance directives. Coordinate referrals to community resources, financial assistance programs, counseling services, and bereavement support. Advocate for patients and families to ensure access to appropriate hospice and community services. Participate in interdisciplinary team meetings as requested. Educate patients and families regarding hospice philosophy and available supportive services. Maintain accurate and timely electronic clinical documentation in compliance with agency policies and state, federal, and Medicare hospice regulations. Maintain patient confidentiality in accordance with HIPAA regulations. Participate in required orientation, annual competencies, and continuing education. Qualifications Required Current Texas Licensed Medical Social Worker (LMSW) license in good standing. Master's Degree in Social Work (MSW) from an accredited school of social work. Valid Texas driver's license. Current automobile insurance. Reliable transportation. Ability to travel throughout the Lewisville service area. Preferred One (1) year or more of experience in hospice, home health, hospital, rehabilitation, skilled nursing, or medical social work. Knowledge of hospice philosophy, end-of-life care, bereavement support, care coordination, and community resources. Knowledge & Skills Successful candidates will demonstrate the ability to: Deliver compassionate, patient- and family-centered care. Perform comprehensive psychosocial assessments. Provide grief counseling and emotional support. Coordinate community resources and supportive services. Build trusting relationships with patients and families. Communicate effectively with patients, families, physicians, and interdisciplinary team members. Exercise sound clinical judgment and professional ethics. Maintain accurate electronic documentation. Demonstrate strong organization, documentation, and time management skills. Adapt quickly to changing patient care needs and scheduling demands. Physical Requirements This position requires the ability to: Stand, walk, bend, kneel, and lift up to 25 pounds with or without reasonable accommodation. Travel throughout the assigned service area between patient homes and healthcare facilities. Respond appropriately to patient and family needs. Provide extended periods of direct patient and family interaction in home and facility settings.
