Linkage to Care Coordinator/MHU Driver

The Linkage to Care Coordinator will drive the process(es) for linking clients to care programs. This will include outreach, intake, referrals of clients to care, harm reduction counseling, and collaboration with Peer Support Specialists to assist clients in achieving substance use/harm reduction goals. The Linkage to Care Coordinator will also serve as a mobile unit driver, facilitating the delivery of services in the community. This dual-role position is crucial to ensuring the safe transportation, setup, and operation of the unit while also providing a secure environment for staff, patients, and equipment when needed. This is a mobile, field-based position with occasional responsibilities at the Arlene Cooper Community Health Center when the unit is not deployed. The role requires flexibility, cultural humility, and the ability to create safe, welcoming environments in diverse community settings. The Linkage to Care Coordinator/MHU Driver also supports stigma reduction efforts and expands access to prevention and care services through outreach and education. Key Responsibilities Demonstrate knowledge of public health strategies to prevent opioid overdoses and related harms. Build relationships with community groups and organizations that provide services needed by clients. Partner with an interdisciplinary healthcare team to drive identification, screening, and enrollment of individuals into MAT, MOUD, and SUD services, ensuring adherence to clinical protocols and eligibility criteria. Perform ongoing case management efforts by coordinating services, monitoring client progress, and maintaining accurate documentation to ensure continuity of care and compliance with program and grant requirements. Conduct rapid HIV testing, rapid HCV testing, and pregnancy testing. Conduct STI screening and risk reduction counseling. Identify at-risk groups to prevent, treat, and reduce the incidence of communicable diseases and risks to others. Perform tests at off-site locations and mobile units; provide risk reduction training to the public; conduct educational talks for small groups. Safely operate the Mobile Health Unit to and from scheduled service locations. Conduct routine inspections before and after each trip (e.g., tire pressure, fuel, fluids, lights). Maintain accurate logs of mileage, fuel usage, and other operational data. Ensure the mobile unit is maintained, clean, and operational, including routine inspections and coordinating any necessary repairs. Always maintain cleanliness and proper upkeep of the vehicle. Navigate efficiently using GPS and follow pre-planned routes or as directed. Assist with setting up and breaking down the mobile unit, including equipment handling. Monitor maintenance schedules and report any mechanical issues promptly. Assist with the loading and unloading of supplies, materials, and equipment for mobile unit operations. Provide and oversee a secure environment during health service delivery, particularly in high-traffic or sensitive areas. Monitor surroundings and assist with crowd control or patient line management if necessary. Support staff with patient interaction protocols, ensuring everyone’s safety and comfort. Respond appropriately to emergencies, including assisting with first aid or contacting law enforcement if needed. Maintain clear communication with site supervisors and respond to security concerns in real time. Work as part of an interdisciplinary team to develop community activities and events to connect clients with service agencies. Research, conduct, and participate in weekly outreach efforts to meet grant deliverables. Conduct peer-to-peer and evidence-based training for the community. Provide services at the Arlene Community Health Center during clinic hours when the mobile unit is not in the field. Further the Center’s goals regarding social justice, civil rights, and LGBTQ+ issues. Collaborate with other substance misuse programs to ensure seamless services for clients. When the MHU is not going out, you will be required to assist in the clinic or support other team members. Perform other duties as assigned. Skills Experience in operating large vehicles, preferably mobile units or vans, with knowledge of safe driving practices. Excellent organizational skills, analytical ability, and strong attention to detail. Ability to adapt and support change management. Strong analytical skills and proficiency in Microsoft Office, EMR, especially eClinicalWorks. Ability to conduct interviews of a highly personal and sensitive nature. Quick learner with the ability to apply new knowledge across different situations. Strong communication, interpersonal, and relationship-building skills. Effective time management and multitasking abilities. Ability to deliver complex information clearly and concisely. Ability to draw blood and administer basic point-of-care testing. Demonstrated ability to prioritize deliverables in a dynamic environment. Education and Work Experience High School Diploma or equivalent required. Valid driver’s license with a clean driving record. Minimum 2 years of professional driving experience (experience with larger vehicles or RVs is a plus). A current CDL is preferred; candidates without a CDL may be considered based on experience and qualifications. Peer Recovery Support Specialist (PRSS) certification is preferred. Candidates with relevant lived experience and a willingness to obtain certification will be strongly considered. Familiarity with harm reduction, syringe access programs, HIV/STI prevention strategies, and community organizing. Experience with mobile or field-based public health outreach. Prior experience in security, public safety, or a similar field preferred. CPR and First Aid certification (or willingness to obtain upon hire). Strong communication skills, professionalism, and ability to work well in a diverse team environment. Flexibility to work varied hours, including early mornings, evenings, and occasional weekends. Compassionate, empathetic, and non-judgmental approach to client support. Fluency in English and Spanish is highly preferred. Ability to work with diverse groups in diverse settings, including in The Center as well as in the community. Physical Requirements Ability to lift, carry, and move equipment and supplies weighing up to 50 lbs. Ability to stand, walk, bend, and reach for extended periods during outreach activities and mobile unit setup/breakdown. Capacity to drive and operate a mobile unit or large vehicle safely for extended periods, including performing pre- and post-trip inspections. Ability to enter and exit the vehicle frequently throughout the day. Comfortable working in unpredictable environments, including outdoor community settings, neighborhoods, and mobile health event sites. Tolerance for variable weather conditions, including heat, cold, or rain, while maintaining client service standards. Ability to communicate effectively in person and over radio/phone while on-site or in the field. Visual and auditory ability sufficient to monitor safety conditions and respond to environmental or client cues. Manual dexterity and steadiness to perform tests.
