Job Overview With a commitment to patient- and family-centered care, the professional nurse uses the nursing process to value the uniqueness of each individual and address the physical, psychological, emotional, and social needs of the diverse patients served by Jefferson Health. The nurse develops nursing plans of care with the patient and their designated care partner(s) to effectively meet individualized needs and priorities consistent with their cultural beliefs and values. Responsibilities Interacts with co‑workers, patients, their family/designated care partner(s), and other staff in a manner consistent with Jefferson values. Integrates Evidence‑Based Practice, clinical expertise, patient preferences, and cultural beliefs into the delivery of optimal health care. Partners with patients and families in identifying learning needs and priorities and provides education that is useful, understandable, and affirming. Minimizes risk of harm by following established nursing practices, protocols, and individual performance standards. Uses information technology to communicate, manage knowledge, mitigate error, and support ethical decision making. Uses data to monitor care process outcomes and applies improvement methods to test changes that continuously improve quality and safety. Functions effectively within nursing and inter‑professional teams, fostering open communication, mutual respect, and shared decision‑making. Recognizes patients and their families in providing safe, compassionate, and coordinated care based on personal preferences, priorities, needs, and cultural values. Creates an environment of belonging and inclusion, demonstrating problem‑solving and conflict‑resolution skills. Demonstrates commitment to learning, seeks opportunities for professional growth, and acts as a primary preceptor when appropriate. Actively participates in hospital‑ and practice setting‑based committees, including safety and quality improvement. Demonstrates willingness to give and receive feedback, integrating changes into practice. Minimum Qualifications Specialized Diploma or accredited school of nursing graduate required. Patient‑and Family‑Centered Practice knowledge and ability to convey respect and collaboration. Cultural awareness and ability to connect effectively with diverse backgrounds. Effective communication skills, understanding of concepts, tools, and techniques for accurate transmission and interpretation. Patient assessment and care knowledge, including implementation of treatment plans. Patient and Designated Care Partner education knowledge and ability to elicit learning priorities. Medication administration and prescription knowledge of various medications. Discharge planning knowledge and ability to partner with patients and families in developing discharge plans. Assessment and diagnosis knowledge of tools and techniques for nursing diagnosis or clinical judgment. Health information documentation knowledge and ability to record medical histories, vital signs, medications, diagnoses, treatment plans, procedures, tests, and results. Licensed Registered Nurse or PA licensed in the State of Pennsylvania. Preferred Qualifications Bachelor’s Degree (BSN) or completion of BSN or appropriate advanced degree within 3 years of hire strongly preferred. One year previous experience as an RN in an acute care setting strongly preferred. Additional Qualifications Physical demands: lift and carry 40‑50 lbs, push/pull patients on bed or stretcher (requires 29 lbs push force), lateral transfers up to 50 lbs of patient weight, frequent to continuous standing/walking, walk 8‑10 miles per shift. May require kneeling, bending, squatting, and performing CPR.
