$34-$36/hour

Responsibilities for Admissions Coordinator:

• Greet new admissions and families and answer any questions that may arise.
• Take patient's photo upon admission.
• Complete admission nursing documentation and assessments.
• Medication Reconciliation
• Review Labs
• Notification of attending and consulting NPs/MDs admission notification and obtain orders.
• Communicate any observations in change of patient's status to the nursing team.
• Work closely with the IDT.
• Attend Family meetings with IDT when needed.
• Obtain POLST form and enter code status in chart.
• Prepare and send daily end of day e-mail to recap to Administrator and DON highlighting the areas that are being addressed.
• Follow up on any issues with service delivery noted by patient, families, etc.
• Document actions by completing forms, reports, and logs as required by facility.
• If needed, assist care team with pass trays, answering call bells, etc.
• Act as a problem solver to address resident or family issues or concerns.
• Embrace our person-centered care approach to meet each patient's needs and preferences.
• Other duties as assigned

Qualifications for Admissions Nurse:

• Minimum 2+ years in a related Admissions or Business Development position within long-term care or related healthcare setting.
• Previous health care community relations experience.
• Must have a strong sense of urgency, self-motivation, and goal orientation.
• Ability to work flexible hours as admissions and marketing responsibilities may dictate
