Physical Therapy Assistant

Job Summary:

We seek a highly skilled and experienced Registered Nurse with specialties in Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, or Operating Room. The Registered Nurse will be responsible for providing skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. Daily tasks include patient assessments, medication administration, treatment planning, and collaborating with the interdisciplinary team. Strong communication and collaboration with patients, providers, and team members are critical. A minimum of 2 years of experience and exceptional skill is required. Responsibilities:

  • Perform patient assessments and develop treatment plans.
  • Administer medications safely and accurately.
  • Provide skilled nursing care in various clinical settings.
  • Manage patient cases including hospice care and coordination.
  • Collaborate with interdisciplinary teams to optimize patient outcomes.
  • Communicate effectively with patients, families, and healthcare providers.
Required Skills & Certifications:
  • Active Registered Nurse license in the state of California.
  • Bachelor's degree in Nursing or related field.
  • Minimum of 2 years hospital, home health, or hospice care experience.
  • Clinical assessment, medication administration, and treatment planning skills.
  • Strong interdisciplinary team collaboration skills.
  • Excellent communication and interpersonal skills.
  • Driver's license, car insurance, and automobile for field work.
Preferred Skills & Certifications:
  • Experience with SNF (Skilled Nursing Facility) settings.
  • Familiarity with NetHealth (Rehab Optima) software.
Special Considerations:
  • Must maintain current California RN license.
  • Ability to work in fast-paced and diverse clinical environments.
  • May require field work and travel within the assigned area.
Scheduling:
  • Work schedule and shifts to be determined based on facility needs.
  • Flexibility to work various shifts including days, nights, or weekends as required.
