Employment Type: Full time Shift: Day Shift Description: Trinity Health Chelsea Hospital Location: Chelsea, MI Schedule: Full-time, Day Shift Looking for an energetic candidate who enjoys working with inpatient population. Schedule is M-F and requires two weekend days per month as well as treating the acute care population. POSITION PURPOSE The Rehabilitation Services Staff Physical Therapist provides evaluation and treatment of patients in a range of specialty programs. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES 1. Maintains good rapport and cooperative relationships. Approaches conflict in a constructive manner. Helps to identify problems, offer solutions, and participate in their resolution. 2. Maintains the confidentiality of information acquired pertaining to patient, physicians, colleagues, and visitors to Chelsea Hospital. Discusses patient and hospital information only among appropriate personnel in appropriately private places. 3. Behaves in accordance with the Code of Conduct, Service Excellence Standards, and the Mission, Vision, Values and policies of Chelsea Hospital. 4. Assumes responsibility for performance of job duties in the safest possible manner, to assure personal safety and that of coworkers, and to report all preventable hazards and unsafe practices immediately to management. 5. Demonstrates self-directed learning. 6. Provides quality patient care by considering the age specific, development and cultural needs through competent clinical practice. Demonstrates unit/area competencies. 7. Identifies and assists in resolution of problems related to safety, sanitation, and maintenance of the environment. 8. Practices effective communication in all interactions (patient, visitor, co-worker, etc.) 9. Promotes a positive work environment by demonstrating teamwork. 10. Demonstrates competency with individualized assessment of patients using a wide variety of assessment measures and procedures. 11. Provides a thorough physical therapy evaluation and creates an appropriate plan of care based on the patient deficits. 12. Stays up to date on all participating insurance guidelines for rehabilitation services. 13. Provides thorough documentation on each patient interaction. Documentation must be timely and include all pertinent information required by state law, accrediting agencies, and 3rd party payers. 14. Formulates individualized, age specific, therapy goals based upon assessment results and patient needs. 15. Consults with other team members and selects the most appropriate plan of care consistent with the needs and capabilities of the patient. 16. Implements patient care in a manner consistent with the therapy goals and the changing needs of the patient, using a variety of therapeutic approaches. 17. Completes daily charges for each patient while adhering to ethical practices per insurance guidelines for each payer. 18. Orients patients and families to the therapeutic purposes and procedures of physical therapy, and educates them about home programs, impairments and progress. 19. Provide daily oversight of the physical therapy provided by staff working under the physical therapist (could be physical therapist assistant, athletic trainer or physical therapy aide) and co-sign documentation when required. 20. Provides re-evaluations/re-assessments of patients on a regular basis to ensure progression towards goals. 21. Supervises Physical Therapy student affiliates as assigned by the Manager or Student Program Coordinator. 22. Provides for own professional growth and education and contributes to staff development activities. 23. Works with Manager and secretarial staff to ensure schedules are managed well to achieve targeted productivity and clinic budget. 24. Acute Care physical therapists that see patients on the IP Rehab Unit will be required to participate in interdisciplinary team meetings 1-2 times per week. 25. Participates in regularly scheduled staff meetings and inservices. OTHER FUNCTIONS AND RESPONSIBILITIES Performs other duties as assigned. REQUIRED EDUCATION, EXPERIENCE AND CERTIFICATION/LICENSURE Education: Bachelors Degree or higher in Physical Therapy from accredited university Experience: Certification/Licensure: Licensed as a Physical Therapist in the State of Michigan REQUIRED SKILLS AND ABILITIES 1.
