Genesis Homecare Agency is seeking an outgoing, motivated, and relationship-driven professional to join our growing team as a Community Outreach & Client Relations Coordinator .

This is an ideal opportunity for someone who enjoys networking, building community relationships, meeting new people, working independently in the field, and helping families connect with quality in-home care.

This is primarily a community outreach and client relations position , with occasional direct-care field support when operationally necessary.

Compensation & Opportunity

• $45,000 annual base salary
• New client enrollment commission
• Earn $100 for every 10 weekly service hours secured at intake
• Example: A new client enrolling for 40 hours per week = $400 commission
• Eligible private-pay, long-term care insurance, and VA clients may qualify for commission under the company's commission plan
• Full-time position
• Primarily Monday–Friday during normal business hours
• Local travel throughout the assigned service area

What You'll Do
Community Outreach & Business Development

• Build relationships with senior communities, healthcare providers, rehabilitation facilities, social workers, case managers, churches, veteran resources, and other community partners.
• Conduct scheduled visits with current and prospective referral partners.
• Attend networking events, health fairs, senior events, and community activities.
• Identify new referral and client-development opportunities.
• Follow up consistently with prospective clients, families, and referral sources.
• Maintain accurate records of outreach activity, referrals, leads, and follow-ups.
• Work toward established monthly client-growth goals.

Client Relations & Start-of-Care Support

• Conduct nonclinical in-home client assessments and intake visits.
• Help prospective clients and families understand Genesis Homecare Agency's services and start-of-care process.
• Conduct caregiver/client meet-and-greets before services begin.
• Help ensure a smooth transition from client enrollment to active services.
• Complete supervisor handoffs and communicate important information to the office team.
• Conduct client follow-ups and satisfaction visits.
• Maintain timely and accurate documentation of assessments, visits, and client interactions.

Occasional Field Support

Because this is a field-based home care position, the Coordinator must also be comfortable providing occasional authorized direct-care support when needed to help stabilize a new case or address an urgent coverage need.

Any direct-care services must be performed in accordance with the client's service plan and Genesis Homecare Agency policies.

Requirements:Who We're Looking For

We're especially interested in candidates with previous experience in:

• Home care or home health
• Hospice
• Senior living or assisted living
• Healthcare marketing
• Community outreach
• Admissions or client relations
• Business development or sales
• CNA, PCA, or professional caregiving

You should be personable, confident, organized, persistent, goal-oriented, and comfortable starting conversations and developing professional relationships.

Requirements

• Strong communication and relationship-building skills
• Comfortable working toward clearly defined monthly client-growth goals
• Ability to work independently and manage a field-based schedule
• Valid driver's license, reliable transportation, and current automobile insurance
• Ability to travel throughout the assigned service area
• Ability to provide occasional direct personal-care assistance when needed
• Must meet Genesis Homecare Agency caregiver onboarding requirements
• Current CPR/First Aid certification or willingness to obtain certification
• Successful completion of required background checks and health screenings
• Comfortable using smartphones, email, electronic documentation, and scheduling systems
• AxisCare experience is a plus, but not required

Schedule

This is a full-time, primarily Monday–Friday field position . Most work will occur during normal business hours.

Occasional evenings or weekends may be necessary for scheduled community events, client meet-and-greets, or exceptional operational needs.
