Job Title: Advanced Practice Wound Care Nurse Practitioner (AGNP/CNP) | Geriatric Rounding Compensation: $120,000 to $160,000+ Total Compensation (Base Salary + Productivity Incentive Program) + Mileage Reimbursement Benefits: Full Benefits Package, PTO, Paid Holidays, CME Allowance, Malpractice Coverage, DEA Reimbursement, and More Schedule: Full-Time | Monday through Friday Most healthcare providers rarely get to see the full outcome of their work. What You'll Be Doing Provide advanced wound care assessments, treatment planning, and ongoing wound management. Conduct routine rounding visits within assisted living, memory care, skilled nursing, and long-term care communities. Manage medications, diagnostics, treatment plans, and follow-up care. Collaborate with physicians, nurses, therapists, caregivers, and community staff. Support chronic disease management and continuity of care initiatives. Help improve healing outcomes while reducing avoidable complications and hospitalizations. Qualifications Active Minnesota Certified Nurse Practitioner (CNP) license (AGNP preferred). Experience in wound care, geriatrics, primary care, post-acute care, or skilled nursing preferred. Strong clinical judgment and confidence managing medically complex patients. Valid driver's license and ability to travel throughout the Twin Cities Metro Area. Some providers spend their careers hoping their interventions worked.
