NURSE PRACTITIONER

Signify Health is seeking an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations for Medicare and vulnerable populations in their homes. In this field-based role within our Healthcare & Medical Services department, you’ll review histories, medications, and social factors; develop evidence-based care plans; and collaborate with primary care and care coordination teams. You’ll leverage modern technology, EHRs, and data-driven insights to support value-based care, improve outcomes, and help patients manage chronic conditions while enjoying a flexible, mission-focused, and collaborative culture.
