MDS Nurse (LVN)

MDS NurseWe are seeking a highly skilled and experienced MDS Nurse to join our team at West Haven Healthcare in West Covina, California. As an MDS Nurse, you will be responsible for developing treatment plans for residents that comply with MDS guidelines, ensuring accurate and timely submissions, and maintaining the confidentiality of all resident care information.ResponsibilitiesConducts and coordinates the development and completion of the Minimum Data Set (MDS) in accordance with current rules, regulations, and guidelinesMaintains MDS schedule and informs social workers of pending MDS assessmentsGenerates and distributes monthly MDS calendarCreates new assessments according to MDS scheduleSubmits the MDS report in a timely fashion to meet established reporting deadlinesEnsures the accuracy of the MDS reportEnsures that each portion of the assessment is signed and dated by the person completing that portion of the MDSCreates and transmits any MDS assessments that need correctionEnsures that the MDS report is compliant with current regulationsKeeps abreast of changing MDS requirements and implements changes as necessaryMaintains and periodically updates written policies and procedures that govern the development, use, and implementation of the resident assessment (MDS) and care planInforms Administration and other departments affected by changing MDS requirementsAssesses charts and communicates with healthcare teams to create applicable healthcare plans for current and incoming residentsPerforms administrative duties as necessaryCoordinates the shipment of facility's services to residing residentsObserves and documents pricing and effectiveness of servicesConducts new resident assessments to help stabilize and improve practices of the facilityProvides OBRA assessments to ensure compliance with Medicaid or Medicare standardsActs as the main communicator between insurance professionals and facility managersWorks well with other departments to develop good rapport and to assure communication of resident needs and goalsMeets with Director of Nursing regularly to discuss policies and procedures, rules and regulations, standards, department objectives, and philosophy, problems in patient care, MDS, and care planning, etc.RequirementsMust possess a Nursing degree from an accredited nursing programMust be licensed as a Registered Nurse in the State of CaliforniaMust possess MDS Certification from an accredited programMDS experience in a Long-Term care settingAbility to speak, read, and write English effectivelyEmotional maturity, reliability, tact, integrity, and good judgmentGood problem-solving skills with ability to plan, organize, and execute work activities independentlyAbility to interact effectively with team members, residents, families, and visitorsPhysical Demands/Work EnvironmentThe demands and environment described here are representative of those that must be met by an employee to successfully perform the functions of this jobReasonable accommodations may be made to enable individuals with disabilities to perform the essential functionsWork primarily involves standing, walking, twisting, reaching, bending/stooping, as it relates to standard office activityNoise level in the facility work environment is usually moderately quiet, but may be occasionally loud due to resident outburstsSalary: $40 - $46 HOURLYJob Type: FULL TIMEMinimum Years of Experience: 1BenefitsMedical, Dental & Vision insuranceLife InsuranceSame day pay (Tapcheck)401(k) Retirement PlanTime Off/Leave (PTO, Vacation or Sick Leave)Holiday PayPaid orientationIf you are a highly motivated and experienced MDS Nurse looking for a new challenge, please apply to this role.
