Position Overview

This role involves providing therapy services through home health visits. The position requires reliable transportation, a current driver's license, and agency-required liability insurance.

Shift Information

  • Shift Type: Day 5x8-Hour (08:00 - 17:00)
  • Contract Duration: 13 weeks
  • Weekend Requirements: Varies per location
  • On Call Requirements: Varies per job and location
Responsibilities
  • Conduct home visits for therapy services.
  • Participate in periodic office meetings.
  • Document patient visits and activities promptly, completing all documentation within 24 hours.
  • Accept assigned patients and manage travel between appointments effectively.
  • Maintain a weekly productivity expectation of 30 points through patient visits and documentation.
Patient Care
  • Average Number of Patients per Day: 5-6
  • Types of Cases: Start of Care and Follow Ups
Qualifications
  • Experience: Minimum of 1 year of recent adult home health experience required.
  • Certifications: CPR and BLS are required.
  • License: Must possess a valid license.
  • Technology: Proficiency in using electronic medical record (EMR) systems, specifically HomeCare HomeBase and OASIS.
Travel Requirements

The contractor must be willing to travel up to 75 miles from the designated home clinic. Mileage will be reimbursed at the IRS standard rate for travel between patients.

Additional Information
  • Local candidates are welcome to apply.
  • Contractor will have the opportunity to float to nearby clinics if patient census is low.

Location

Position requires coverage primarily in Benton and Franklin counties.
