Care Coordinator

The Care Coordinator provides community-based behavioral health services to children and families, delivering individualized support across home, school, office, and community settings. This role considers each family's unique needs, strengths, cultural preferences, and community environment to provide meaningful, client-centered care.

The Care Coordinator provides direct behavioral health services, supports crisis management, develops and coordinates individualized service plans, connects families with community resources, and collaborates with internal and external providers to promote continuity of care. This position also includes documentation and administrative responsibilities and requires a strong commitment to evidence-based practices, professional development, and Best Point's standards of care.

Key Responsibilities

Direct Client Services

• Provide behavioral health interventions, including needs assessments, crisis management, and skill development.
• Develop and coordinate Individualized Service Plans (ISPs) in collaboration with clients, families, and service providers.
• Provide services that promote skill development and generalization across home, school, and community settings.
• Support clients and families during transitions between care systems.
• Assist clients in accessing appropriate community resources, services, and support systems.

Collaboration & Care Coordination

• Facilitate team meetings and coordinate treatment with internal and external providers.
• Communicate effectively with clients, families, schools, community partners, and other service providers.
• Promote continuity of care through consistent communication and coordinated services.
• Consider family dynamics, cultural preferences, and community environments when developing and delivering services.

Documentation & Professional Practice

• Maintain timely, accurate, and thorough clinical documentation.
• Prepare meeting agendas, document meeting notes, and complete required administrative tasks.
• Adhere to agency, regulatory, HIPAA, and program standards.
• Participate in professional development, training, and ongoing skill-building.
• Provide interventions based on best practices and participate in quality improvement and process improvement activities.

Qualifications

• Bachelor's degree in Social Work, Counseling, Psychology, or a related field from an accredited institution.
• Strong documentation, organization, and time management skills.
• Ability to work independently while also collaborating effectively as part of a multidisciplinary team.
• Strong communication, interpersonal, problem-solving, and crisis-management skills.
• Ability to travel throughout the community to provide services in various settings.
• Valid driver's license and ability to safely operate a motor vehicle in changing weather and community conditions.

Compensation & Benefits

Pay: $20.00–$22.00 per hour, based on qualifications and experience.

Work Environment

This position works across office, community, school, and home environments and involves providing services to children, families, and adults with behavioral health needs. Employees may encounter individuals experiencing behavioral or mental health challenges and may be exposed to shouting or verbally or physically aggressive behaviors.
