Key Responsibilities Conduct thorough psychosocial assessments and develop personalized treatment plans in conjunction with the healthcare team, documenting patient progress and making adjustments as necessary. Engage in open communication with physicians and other healthcare providers, proactively discussing patient needs, progress, and any emerging concerns to optimize care delivery. Maintain accurate and timely clinical documentation, including progress notes and discharge summaries, using electronic health records to ensure compliance with regulatory standards. Provide crisis intervention and therapeutic support to patients experiencing emotional and behavioral difficulties, employing evidence-based modalities and interventions. Facilitate coordination with clients' family members and support systems to ensure a comprehensive and supportive approach to treatment and discharge planning. Patient Population / Client Focus The client population served by Expression Behavioral Healthcare includes individuals facing a range of mental health challenges, from depression and anxiety to more complex behavioral issues. Qualifications Master’s degree in Social Work from an accredited institution, with specialized training in counseling and clinical practice. Current and active state licensure as a Licensed Clinical Social Worker (LCSW). A minimum of 3 years of hands-on experience in an outpatient clinical setting, demonstrating your proficiency in providing therapy and treatment to diverse populations. Proficient in using standard computer applications including MS Office, email systems, and electronic medical records. Strong organizational skills with the ability to work independently, manage multiple cases, and prioritize responsibilities effectively. Preferred Experience Experience with telehealth services, particularly in a remote work environment, is highly desirable. Familiarity with evidence-based practices, therapeutic techniques, and integrative care approaches will be an asset in this role. Experience in crisis intervention and management, particularly in a community setting. Compensation and Benefits The compensation for this full-time position ranges from $90,000 to $100,000 annually, reflecting your qualifications and experience.
