Schedule

5x8 Day Shift (08:00 AM - 04:30 PM), Monday through Friday. No weekends or holidays required.

Job Description

The Case Manager RN will be responsible for managing care for all medical and surgical patients across various units, including ICU and IMC. This role emphasizes care coordination, discharge planning, and ensuring compliance with CMS guidelines.

Responsibilities

• Case management of all medical and surgical patients
• Coordinating care and facilitating discharge planning
• Conducting continued stay reviews and determining medical necessity
• Managing appeals and denials related to patient care
• Collaborating with interdisciplinary teams for optimal patient outcomes

Required Experience / Certifications / Licensure

• Active RN license in MD or Compact state
• Current BLS certification
• Care Management certification preferred (CPI certification required)
• Minimum of 2 years of charge experience in a similar role
• Experience with Epic charting system required
