Physical Therapist Assistant – PRN The Hills Post Acute – Santa Ana, CA The Hills Post Acute is a post‑acute care facility that provides long‑term, short‑term, and geriatric services. We offer a supportive, in‑house therapy team and well‑structured rehab programs for our geriatric clientele. We are seeking a dedicated PRN Physical Therapist Assistant for our in‑house rehab program. Key Responsibilities Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards. Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements. Perform regular standing and walking throughout the facility; occasionally lift, transfer, and reposition patients and equipment as necessary. Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care. Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders. Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements. Qualifications Physical Therapist Assistant license is required. Open to Physical Therapist Assistants at all experience levels; new graduates are welcome.
