Job Summary

Our client is seeking a Home Health Licensed Practical Nurse to manage and oversee patient care, execute skilled nursing procedures, and provide caregiver training and mentorship.

Key Responsibilities

• Clinical Case Management: Manage assigned patient cases, oversee care delivery, and regularly evaluate patient needs (~4 to 5 patients per day).
• Medication & Skilled Care: Execute skilled nursing procedures, including medication administration, ostomy care, and catheter care.
• Caregiver Training & Mentorship: Train, guide, and oversee caregivers (~70 caregivers total) on non-skilled procedures such as client transfers, bedbound repositioning, toileting, and bathing.
• Onboarding & Introductions: Conduct client introductions and ensure smooth onboarding for care plans.
• Field & Office Support: Assist with community marketing efforts, general office tasks, and collaborative operational workflows alongside case management and scheduling teams.

Qualifications

• Active Licensed Practical Nurse (LPN) license in good standing.
• Hands-on experience with medication management and skilled nursing procedures (catheter and ostomy care required).
• Strong situational judgment, alignment with team culture, and demonstrated stability/longevity in previous roles.
• Comfortable leading caregiver training and guiding teams on non-skilled care delivery.
• Experience in Home Health is preferred.

Experience

• 5+ years of LPN experience desired
