Role Overview The Nurse Practitioner provides effective and equitable primary care, sees patients independently, and collaborates closely with physicians and the multidisciplinary care team. The role emphasizes longitudinal care, patient relationship building, and participation in population health initiatives. Core Responsibilities Provision of exceptional primary care. Conduct office visits for routine and acute issues. Administer annual wellness visits and health risk assessments, requiring a holistic view of health and meticulous documentation. Coordinate care with other providers, specialists, testing facilities, and agencies. Lead population health initiatives in collaboration with the Care Team, such as ensuring eligible females receive evidence‑based breast cancer screening every two years. Assist the care team with phone triage and outreach. Educate patients on their health conditions, care plans, and treatments. Participate in Oak Street Health promotional activities. Conduct home visits as needed. Other duties as assigned. Required Qualifications Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate. National certification in at least one specialty: Family Nurse Practitioner, Adult‑Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner. Actively licensed Nurse Practitioner in the state, non‑probationary. Active DEA license. U.S. work authorization. Preferred Qualifications Experience in primary care: internal medicine, geriatrics, or family medicine. Knowledge of HCC (Hierarchical Condition Categories) documentation, ICD‑10 coding, and health risk assessments. Passion for teamwork and cross‑functional collaboration. Desire to be part of an empirically‑guided population health model. Bilingual proficiency in applicable areas. Additional Details Anticipated weekly hours: 40. Employment is full‑time. Pay range for this position is $78,331.00 – $168,714.00; the actual offer will depend on experience, education, geography, and other factors.
