Occupational Therapist (OT) – PRN

We are seeking a compassionate, skilled, and motivated Occupational Therapist (OT) to join our rehabilitation team on a PRN basis . In this role, you will provide high-quality, individualized occupational therapy services designed to help residents regain function, increase independence, improve safety, and achieve meaningful quality-of-life goals.

• Conduct comprehensive occupational therapy evaluations and identify residents’ functional needs.

• Develop individualized treatment plans based on clinical findings, physician orders, resident goals, and care plans.

• Provide skilled therapy interventions to improve strength, mobility, coordination, balance, cognition, communication, activities of daily living, and overall functional independence.

• Monitor resident progress and adjust treatment approaches based on clinical response and changing needs.

• Complete accurate, timely, and compliant documentation, including evaluations, daily treatment notes, progress reports, and discharge summaries.

• Collaborate with nursing, physicians, physical and speech therapists, social services, and other members of the interdisciplinary team.

• Participate in care planning, utilization review, and interdisciplinary meetings as appropriate.

• Educate residents, families, caregivers, and facility staff regarding therapy goals, interventions, safety strategies, and recommendations.

• Promote resident dignity, safety, independence, and quality of life throughout the rehabilitation process.

• Maintain productivity, documentation, and quality standards.

• Support facility compliance with applicable clinical, regulatory, and documentation requirements.

• Graduate of an accredited Occupational Therapy program.

• Current and active Occupational Therapist license in the applicable state.

• Strong clinical assessment, treatment-planning, and documentation skills.

• Excellent communication and interpersonal skills.

• Ability to work effectively with residents, families, caregivers, and interdisciplinary team members.

• Long-term care, skilled nursing, or geriatric rehabilitation experience preferred.

• Dependable, compassionate, and committed to providing high-quality resident care.

• Ability to work independently while contributing positively to a collaborative rehabilitation team.

• Ability to stand, walk, bend, reach, and perform other activities required during therapy sessions.

• Ability to safely assist, transfer, position, and support residents as clinically necessary.

• Ability to work effectively in a clinical and long-term care environment.
