Clinical Pharmacist

Job Description Summary The Clinical Pharmacist is a licensed professional who is responsible for providing safe and appropriate pharmacy services to Option Care patients in accordance with the policies and procedures of Option Care, pharmacy practice professional standards, and applicable regulatory entities. This entry‑level position introduces the practice of clinical pharmacy in the alternate setting. The Clinical Pharmacist coordinates patient services with the Nursing Department, monitors patient progress toward goals, and educates patients, caregivers, staff and other professionals regarding appropriate medication use. Job Responsibilities Determines patient suitability for home care and organizes all patient‑specific information needed for the assessment, ensuring medical condition and prescribed medication therapy are appropriate for home care services and prognosis with clearly defined outcome goals. Designs, recommends, monitors and evaluates patient‑specific pharmacy care plans for home care pharmacotherapy, recommends modifications to prescribed medication therapy or monitoring parameters when appropriate, and collects patient information to prevent, detect, and resolve medication‑related problems. Modifies pharmacy care plans based on evaluation of monitoring data and other pertinent patient information, applying pharmacokinetic principles and assessing laboratory values. Provides medication‑use education to Option Care staff, other professionals, home care patients, and caregivers, using effective patient education techniques. Ensures continuity of pharmaceutical care across all patient‑care settings, communicates pertinent patient information, prepares and dispenses medications following Option Care policies and procedures, and adheres to state/federal laws and USP standards. Manages and troubleshoots medication administration equipment, maintains and programs equipment used by Option Care, and reports discrepancies in inventory systems. Responds promptly to drug information requests from healthcare providers and patients, formulates evidence‑based responses, and attends educational seminars or CEU opportunities to maintain professional competency. Manages investigational drug products according to regulations and Option Care policies, utilizes pharmacy support personnel effectively, prioritizes workload, and ensures accuracy of work. Complies with accreditation, legal, regulatory, and safety requirements in all activities. Maintains confidentiality of patient and proprietary information, observing legal and ethical guidelines for safeguarding sensitive data. Supervisory Responsibilities No Basic Education and/or Experience Requirements Graduate of an accredited School of Pharmacy. Current, active Pharmacy license as granted by the State Board of Pharmacy. One of the following: (a) At least one year of experience in sterile IV admixture procedures, (b) knowledge of non‑sterile compounding, immunizations and medication therapy management (MTM), (c) completion of an ASHP accredited Pharmacy Residency Program, (d) completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy, or (e) equivalent experience approved by the VP or SVP of Clinical Services. Basic Qualifications Experience providing customer service to internal and external customers, meeting quality standards for services and evaluating customer satisfaction. Basic level skill in Microsoft Word, Excel and PowerPoint, including document creation, formatting, and basic data manipulation. Travel Requirements Willing to travel up to 5 % of the time for business purposes within the state. Preferred Qualifications & Interests Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy. Experience in home care, hospital or other clean‑room setting. Management Requirements At least 1 year of experience in indirect management of team members, including assisting in the development, training and assignment of work/projects to other team members. May perform other duties as assigned. Salary Pay range is $97,113.10 – $161,862.67. Salary will be determined by the applicant’s education, experience, knowledge, skills, and abilities, as well as internal equity and alignment with market data.
