divh2Pediatric Hospice RN Case Manager/h2pThe Hospice Pediatric RN Case Manager will work with our Pediatric team. Primarily responsible for providing professional nursing care to assigned patients. With input from the interdisciplinary team, patient and family, assesses, develops, implements and evaluates the plan of care. We place a high importance on our employees and reward staff in several ways such as:/pulliCompetitive hospice industry compensation/liliBenefits package with multiple plan offerings and generous employer contribution/lili401(k) Retirement plan with employer match/liliSupportive work culture which encourages work life balance/liliSubstantial Paid Time Off, paid holidays birthday benefit/liliEmployee development program managed by in-house education department to support individualized orientation and professional growth needs./liliMonthly mileage reimbursement/liliEmployee Referral Program/liliBusiness casual attire for office staff/li/ulpJOB DUTIES/KNOWLEDGE/JOB PERFORMANCE:/pulliDevelops written plan of care in coordination with an interdisciplinary Team (IDT)./liliOversees implementation of plan, evaluates and changes plan as necessary to meet patient/family needs./liliResponsible for the direction of care provided by LVN and HA in accordance with patients plan of care./liliResponsible for the supervision of services provided by the LVN and HA per TEH policy and regulatory requirements./liliCommunicates all changes to IDT and primary MD./liliPerforms job duties in clinical areas as stated in clinical competency checklist, policies/procedures, standards of care, and critical competencies based on Scope of Practice./liliMaintains timely patient documentation using point care electronic charting./liliImplements and maintains established hospice policies and procedures as well as Standards of Care and regulatory requirements./liliProvides health care instruction to patient and family members./liliOrients, instructs, and trains assigned personnel./liliParticipates in providing 24 hour, seven days a week coverage to patients/families (on-call and census fluctuation)./liliPerforms admission assessment and admission when necessary./li/ulpDEGREE OF TRAVEL:/pulliTravel required for patient visits./liliOccasional travel for educational purposes./li/ulpQualifications:/pulliMust have Associates Degree, Bachelor Degree preferred./liliMust have one year of acute or hospice pediatric experience/liliAt least 12 months experience with 1-2 years preferred in acute care./liliCurrent CA RN license./liliCurrent CA drivers license with proof of insurance./liliCurrent CPR/BLS./liliPrefer certification as hospice nurse./liliPediatric experience preferred./liliProof of recent TB test result./liliBilingual preferred./liliComputer skills (word processing), use of adding machine, calculator, fax, photocopier./li/ul/div
