Physical Therapist (PT) specializing in Home Health (HH) in a local setting. This position involves providing outpatient therapy services within patients' homes, focusing on improving mobility, strength, and functional independence. The role requires strong clinical expertise, independent work capacity, and excellent communication skills to collaborate with interdisciplinary teams and patients.

Job Details

• Schedule: 8-hour shifts, minimum of 40 hours per week, starting on 10/05/2026 and ending on 01/04/2027.
• Required Qualifications: Valid California Physical Therapist (PT) license; Basic Life Support (BLS); valid drivers license; auto insurance; experience with Electronic Practice Information Charting (EPIC) and Oasis.
• Preferred Qualifications: Prior home health experience including at least 1 year of clinical practice; strong skills in patient assessment, mobility training, and developing treatment protocols; ability to educate patients and develop training materials.
• Additional Information: The candidate will conduct approximately 5 patient visits per day, covering clients in South Marin County including towns like Sausalito, Tiburon, Belvedere, and Mill Valley. Must demonstrate critical thinking, effective communication, and clinical problem-solving skills, with the ability to work independently in a demanding environment. Must be proficient in relevant computer systems and possess the ability to analyze clinical data for workflow and practice improvement.
• Certifications: AHA or ARC certifications required.
